Round Health is no longer in the App Store. If you used it for medication reminders, the closest replacement is Apple Health's Medications section with Follow Up Reminders turned on. If you also take injectables, DoneDose is the only option below that tracks injection sites.

What You're Replacing
Round Health's strength was schedule windows: a reminder that fires and a window in which the dose still counts as on time. No app below copies that exactly. What they offer instead is a follow-up ping when a dose is still unlogged, which handles the same failure, a dose you meant to take and drifted past.

Apple Health (Medications)
Built into iOS 16 and later. It handles schedules, lets you log each dose as taken or skipped, and flags potential interactions between the drugs on your list. With Follow Up Reminders on, it pings you again if a dose isn't logged 30 minutes after its scheduled time, with an optional Critical Alert that sounds even when your phone is muted. You can share your medication list with a family member through Health Sharing, and it works on Apple Watch.
Limitation: iPhone only. It has no concept of injections, so no site rotation and no route-specific logging.
MyTherapy
Free, runs on iOS and Android, and supports inhalers and measurements as well as pills. Adherence reports can be emailed to a provider. It's the strongest free option if you're not on an iPhone or you want cross-platform.
Limitation: Injection logging is basic and the interface is functional rather than polished.
Medisafe

If it's only pills and you have an iPhone, start with Apple Health: it's already on your phone and covers the follow-up reminder. If it's pills and you need Android or caregiver visibility, use MyTherapy, Medisafe, or EveryDose. If injections are part of your regimen, use an app that tracks where you injected, because the others will make you keep a second log.
